Several anatomic variations of the lateral nasal wall including different types of the middle turbinates have been described in the rhinological literature. In this study, we reported a unique variant of the middle turbinate, a large concha bullosa inside a giant concha bullosa as a cause of severely impaired nasal breathing. Furthermore, we commented on the embryological development and the pneumatization of the middle turbinate and discussed the differential diagnosis of the variations of middle turbinate respecting the existing body of literature on the subject.
